Hi, I had the same thing and found out that the TurboSIM was already programmed and can not be programmed twice.
To solve this problem you have to reset the TurboSIM. You need the Tpin that is on delivered together with your TurboSIM. (with me it was on the white sticker inside the package where the TurboSim was inside)

Place your original SIM (so not the AT&T SIM) in a other telephone then the iPhone. Ensure that the pin code is activated. If not activate the pincode.

Turn of the phone and place this SIM together with the TurboSIM in that telephone.

The phone will ask for a PIN code, type in the Tpin that was delivered with your TurboSIM. You will get an message that says its the wrong PIN but thats ok. The second time you can enter the correct code. and the power down the telephone and remove the TurboSIM and your original SIM.

Your TurboSIM should be cleared now and you should be able to program the it again.
